Review on My Name Is Khan

To write a review about a film which contains not only the magical combo of Shahrukh Khan-Kajol-Karan Johar but also something different from melodramatic love story or usual remake of hardcore commercial film is pleasurable. My Name Is Khan has a excellent point to a class of audience who likes to watch Shahrukh Khan in a different characteristic. But the film does not show the excellence enough to get a perfect worship from all audiences and critics.In the movie Rizwan embarks on a touching journey and gets the true recognition of his love,love for Mandira_we reveal a simple but touchy characteristic of an ordinary man’s love,we go through an extra ordinary journey. The movie has some points for which it can be appreciated. Firstly, though such story is not brand new but it has a fresh and own style of presence. Though it can not pass through the mark, made by movie like Forrest Gump but the point is it is not bullshit in any sence. MNIK also gives you a massage and create a different and sometime authentic feelings while you are watching.Secondly,the script makes you feel for the character. It is not Shahrukh Khan,it is Rizwan Khan for whom you will think. It is not chiffon sarees or bubbly dialogue for which you will adore Kajol,it is Mandira. It is a woman who really reflect the motherly affection, a loving and caring wife. Thirdly, the tempo of the movie. It has many subplot but they are quite interesting. It’s an exceptionally relevant to a concept-the unity of nations is of paramount importance, We are all coming together to fight for a common cause – called humanity.Everyone knows that terrorism has no religion and it will never have a religion. As a film, it tries to show social responsibility and the character tries to fulfill that from their position. Though the specificity of activities is not beyond question. Fourthly, the movie can clear it’s idea. It’s not all about a disabled man’s fight against disabil ity. It’s a disabled man’s fight against the disability that exists in the world—terrorism, hatred, fighting. My Name is Khan is also about Islam and the way the world looks at Islam but it not takes any sides.It only tries to say that there are only good people and bad people. There are no good Hindus, bad Hindus, good Christians, bad Christians. Either you are a good person or a bad person. Religion is not the criterion, humanity is. Fifthly, all the actors and actresses has done well. Specially Shahrukh-Kajal combo has got another dimension in bollywood after this movie. Kajal has done very well. Her maturity is used very consciously. We get Shahrukh in a new and pleasant avatar after Chak De! India and Swades. Zarina Wahab is very good as Rizwan’s mom.Thanks Shahrukh for making the character more closer to the audience. Yes we can say he can act. Sixthly, the director has used the complexity and complication provoked by rational and regional aspect th ough it is not totally agreeable in some sense. Moreover sometimes there are many reasons behind any happenings like- 9/11, which can not be described so easily. Moreover, Indian Film Indusrtry don’t have the trend to take a risk by making film relevant to such touchy issues though films like New York ,Kabul Express,Main hoon Na had made.But they were not totally focused. I am not telling that MNIK is all focused but it has got some solid point to relate with a serious issue. Use of metaphorical shot is also notable. Specially Rizwan holding poster-Repair Almost Anything headlined is a brilliant one. This stands for a meaningful thought. The narrative style of story telling and blending past time and present time is good but continuity error like showing objects which don’t match with the time showed in the movie is not expected.The narrative style may influenced by The Shawshank Redemption and Godfellas. The ending is quite good but it may be more gothic and more auth entic. The journey of Khan may be showed more smartly in accordance with happenings as sometime it has feel us melodramatic. Cinematography is worthy off a hand of applause, specially using the background and wide angle shot. Editing is fine. Thanks Ravi K. Chandran (director of photography) and Deepa Bhatia (editing) Shankar-Ehsan-Loy has done a great job both in soundtrack and background score.Thanks to the lyricist(Niranjan Iyengar and Javed Akhtar) The songs such as â€Å"Noor-e-Khuda† blend western bar blues and techno sounds with Indian classical styles such as Sufi and Hindustani. The soundtrack is thus representative of the â€Å"indie fusion genre†. Unlike Karan Johar's other films, this film has no lip-sync songs. All the songs are in background. â€Å"Tere Naina† is a good one. In short The content of My Name Is Khan is diametrically opposite to whatever Karan Johar done in the past. My Name Is Khan is â€Å"not all about terrorism, or 9/11.Itâ€℠¢s about a relationship between two people, between an individual and the State, and between an individual and the country. In short, there the three important components: love story, Islam and a mild form of autism. You can watch it. Sometime it is impressive, dramatic, a little bit slow, inspiring, may be controversial but enjoyable too. The pick up line is – There are only two kinds of people in this world. Good people who do good deeds. And bad people who do bad. That's the only difference in human beings. There's no other difference.

Eastern Woodland And The Seven Years’ War The Eastern Woodland Indians mainly consisted of two major regions the Iroquois, which comprised of five tribes and added an additional a sixth later, and the Cherokee. The Indians in the Eastern Woodland nation lived East of the plains and all the way to the coast, Iroquois in North Eastern currently know as the Ohio area and Cherokee South Eastern currently known as the Tennessee and Georgia area. All Indians lived off the lands hunting, gathering, farming, and fishing all to survive.Men constructed bows and arrows to hunt deer and smaller game, women cultivated garden plots gathering corn, beans, and tobacco. The Seven Years’ War or also know as the French and Indian war, the war was fought between Great Britain and France during the years 1756-1763. Warfare was fought in North Eastern America, involving Indians fighting on both sides aligning beside and against European militias. The outcome entailed the British winning the war and with the assistance of the Indians, the French withdrew and were conquered by British dominance.The central purpose for the Indians was to safeguard their homeland and preserve the land independent of foreign dominance. This is why the Seven Year’ War was a pivotal point in Indian civilization because they displayed that they could hold their North Eastern Land. War was eminent with the Indians fighting themselves and siding on different sides. The British constituently pushed for expansion invading the lands of the Iroquois. War and diplomacy involved Europeans and Indians engaging in negotiation to achieve peace trade and land through a diplomacy and not war.War for America the battle between Britain and France continued to propagate because the desire for more land. A fall in demand or underconsumption and overinvestment results in an economic bubble and coupled with incompetence of government policies resulted in lack of confidence (Frank & Bernanke, 8). The lack of confidence resulted in decline in consumption spending as well as investment spending causing panic among bankers and deflation. The investors found it more profitable to hold money rather than invest as profits were declining hence reacted by keeping clear of markets leading to low economic activity. Low activity leads to unemployment and loss of income thus aggravating the situation due to reduced demand. The decline in prices also meant that consumers could buy a lot of goods with less money hence they did not demand more of the goods leading to drop in demand. This causes a recession which refers to a period of economic downturn as a result in reduced a ggregate demand. A prolonged recession leads to a depression. The monetarists on the other hand, explain that the depression was as a result of ordinary recession. In the business cycle, recessions do occur and are necessary to stimulate the economy but it is government policies undertaken at such a time that worsen the situation. The monetarists thus believe that policy mistakes by the monetary policy authorities were the cause of the depression. It is a good example of renaissance art as it carries a religious theme and well depicts foreshortening, the use of chiaroscuro and atmospheric and linear perspective. Chiaroscuro is a technique in which paint is applied in varying tones to show variation in light. Renaissance artists used this technique greatly to show three-dimensionality in their work. Foreshortening further provides the painting with three-dimensionality whereby linear and atmospheric perspectives come in to not only make it three-dimensional but also give it a more realistic look. Managing Diversity Essay Various literatures indicate that managing diversity within today’s labor force has become a primary concern for companies and organizations today. According to the Bureau of Labor Statistics (2012), compared with the labor force of past decades, today’s labor force is older, more racially and ethnically diverse, and composed of more women. Over the next decade, the labor force will become even more racially and ethnically diverse (Bureau of Labor Statistics, 2012). Non-Hispanic whites made up 67.5 percent of the U.S. labor force in 2010 (Bureau of Labor Statistics, 2012). Bureau of Labor Statistics (2012), projects that this group will compose 62.3 percent of the labor force in 2020, a decline of 5.2 percent. By 2020, the Hispanic, Asian, and African American labor force are all supposed to make up 18.6, 5.7, and 12.0 percent, respectively (Bureau of Labor Statistics, 2012). The only way that diversity can be managed successfully is in an organizational culture that values diversity (Sidel, 2009). According to Kulik and Roberson (2009), when an organization values diversity, the people work better together. If people don’t work well together, the organization does not work well (Kulik Roberson, 2009). The purpose of this paper is to explain the difference between the terms managing diversity and valuing diversity, and the four key components that make up diversity management. Differences between Managing Diversity and Valuing Diversity In order to better understand how diversity management fits into an organizational culture that values diversity, certain distinctions must first be made. One must know the difference between diversity management and culture of diversity. Diversity management involves establishing long term goals to develop, promote, and utilize the skills of a diverse workforce (Marquis, Lim, Scott, 2008). By culture of diversity, it is meant that an institutional environment is built on the values of fairness, diversity, mutual respect, understand, and cooperation (Loden Rosener, 1991). A culture that values diversity emphasizes the importance of employees of different races, cultures, ages, genders, sexual orientations, values, beliefs, ethics, and abilities to work together effectively (Lussier, 2012). Diversity Management with an Emphasis on Valuing Diversity There are four main aspects of diversity management are the support of top management, diversity leadership, policies and procedures, and diversity  training (Lussier, 2012). There must be an emphasis of valuing diversity into each of the four aspects of diversity management: management support, diversity leadership, policies, and training. Management Support Diversity management begins with support from top management. While valuing diversity most often begins at the bottom in organizations, it must involve those at the top if it is to succeed long term (Loden Rosener, 1991). Fostering the right organizational culture is one of the most important responsibilities of a chief executive (McDermott, 2001). Billings-Harris and Anderson (2010) state: The CEO’s understanding and willingness to engage and challenge the organization’s leaders with respect to the vision, business strategy, financials, goals, and objectives are critical for survival. This is no less true for the CEO’s involvement with the inclusion and diversity strategy. When led effectively, these initiatives are embedded in the overall strategy and can act as one of a few powerful people-focused catalysts that drive business results. (p.28) Cox (2001), asserts that to ensure management support in diversity initiatives, an organization should create a separate senior executive position focused on diversity objectives. This diversity director should be involved in all aspects of the firm and should attempt to make diversity an overall business requirement (Cox, 2001). Loden and Rosener (1991) state that once diversity is accepted as an organizational value, a new set of assumptions begins to operate within the organization, and that these assumptions are based on the positive contributions that diversity is perceived to offer. As part of a long-term strategy aimed at changing the corporate culture, many executives participate in team building sessions and other activities designed to encourage open dialogue among employees about diversity issues (Lod en Rosener, 1991). Diversity Leadership The second key aspect to diversity management is diversity leadership. Leaders should be able to influence employees to work toward the organization’s objectives (Lussier, 2012). The organization’s leadership should cultivate a mindset that acknowledges that diversity requires long-term cultural change and interprets diversity to include all people (Loden, 1996). Loden (1996) states that rhetorical statements advocating  diversity will not by themselves motivate change, and extensive leadership involvement is needed to help diffuse the principles of diversity throughout the organization and into the attitudes of employees. This involvement requires time and energy on the part of senior leaders and is the most significant sign that diversity is a high priority in an organization (Loden, 1996). Policies and Procedures Policies and procedures of an organization regarding diversity make up the third aspect of diversity management. â€Å"EEO is a legalistic response to workplace discrimination originally mandated by law. Affirmative action programs are outgrowths of equal employment law.† (Loden Rosener, 1991) Loden and Rosener (1991) conclude that while these programs have led to changes in the composition of the labor force, they have not been linked to an organizational culture change. They state that valuing diversity builds on the basic premise of equal employment law and affirmative action, and that by focusing on the quality of the work environment, valuing diversity moves beyond affirmative action (Loden Rosener, 1991). It acknowledges that hiring and promoting diverse people does not automatically lead to mutual respect, cooperation, and true integration (Loden Rosener, 1991). Diversity Training The last aspect of diversity management is diversity training, also sometimes referred to as awareness training. Diversity training programs aim to make people more aware of the issues and the opportunities that exist in reducing differential treatment, including awareness of attitudes behaviors, and biases (Morrison, 1992). According to Morrison (1992), focusing on theories, principles, demographic changes, and organizational benefits of increased diversity is common in diversity training, but it is unlikely to cause individuals to examine their personal values, attitudes, and behaviors. Diversity education is not achieved by one or two workshops, but requires a consistent, continuous effort to understand diversity concepts (Thomas, 2005). Diversity or awareness training for most organizations should consist of three major steps: (1) Begin with leadership education. (2) Follow up with general manager and employee education about stereotyping and the dimensions of diversity. (3) Continue with ongoing seminars in  managing diversity as a vital resource, understanding the dimensions of diversity, career development/efficacy of training for diverse employees, and so on (Loden Rosener, 1991, p. 204). Accountability for Diversity Even though it is not one of the four main aspects of diversity management, it is important to talk about rewarding behavior that values diversity. As efforts to create the culture of diversity evolve, Morrison (1992) states that the three most commonly revised administrative procedures to hold employees accountable are performance evaluations and resulting financial rewards, succession planning, and promotion systems. Employees and management at all levels should be held accountable for nurturing a culture that values diversity, and responsibility must be distributed throughout the organization if diversity goals are to be achieved (Morrison, 1992). Even in cases where employees do not personally endorse the philosophy, their workplace behavior should still be expected to support this change (Loden Rosener, 1991). Conclusions In conclusion, diversity management does not focus on recruitment and hiring practices. Recruitment and hiring polices, such as Equal Employment Opportunity and Affirmative Action, are short term objectives and do not address organizational growth and development (Marquis, Lim, Scott, 2008). Diversity management is a long term focus on developing and utilizing the skills of a divorce labor force and focuses on striving to achieve organizational objectives while creating a positive work environment (Marquis, Lim, Scott, 2008). However, diversity management is only effective if the culture of the organization values diversity (Sidel, 2009). An emphasis on valuing diversity needs to be implemented and supported by top management, and the leaders of an organization need to cultivate the belief and value of diversity (Loden, 1996). Programs and policies must go above and beyond the minimum legal requirements for recruitment and selection (Loden Rosener, 1991). Training and awareness pr ograms should include education about differences in beliefs, cultures, and opinions (Morrison, 1992).

The Benefits of My Fantasy Football League Essay The difference between knowledge and intellect is remarkable, yet many times these words are used interchangeably. Knowledge refers to facts on a given subject; intellect refers to a person’s perspective, how they view, analyze, and interpret their environment . Unlike I.Q., intellect can and should be taught to our students, but instead our current schooling system is focused on ensuring students memorize the facts required to pass an exam. In his essay, â€Å"Hidden Intellectualism†, Gerald Graff explores the limits current education standards impose on our youth’s development. Graff presents the idea that perhaps the subjects that we normally associate with â€Å"anti-intellectualism† are just as capable of being subject of critical thought as Shakespeare’s plays. â€Å"Real intellectuals turn any subject, however lightweight it may seem, into grist for their mill through the thoughtful questions they bring to it† (Graff, 381). This idea is central to understanding the rest of Graff’s argument. If no subject is more deserving than another, then every subject—sports to science—should be utilized in the classroom as learning tools. Young students are motivated in complicated ways. The things that interest them are normally not academic texts of Plato or George Orwell. The author himself identified himself as a person who â€Å"hated books and cared only for sports† (Graff, 381). The only readings that interested him were sports novels and magazines. Over time, Graff developed the idea that his love for sports was not actually anti-intellectual as he had previously assumed, but was as intellectual as his university studies. He claims that had his teachers utilized his love for the workings of the sports world as an outlet to spark academic discussion and thought, he would have earned a stronger education. While Graff was postponing his English homework to have a debate with his best friend about who was the best pitcher in the 50s, he was practicing the skills that would later allow him to become a successful professor at the University of Illinois. Engaging in heated debates about baseball gave Graff opportunities to practice forming coherent arguments supported with evidence he had to collect and analyze while at the same time perfecting his conversation skills and logical thought patterns. What is the goal of elementary education if not to teach one how to learn and think? Sure, one needs to know their multiplication tables and Presidents, but the curriculum should be designed to teach students how to educate themselves in order to think critically about their world. The first step in education, according to Graff, should be providing the pupil with a topic they are interested in. Doing so gives students a bridge from their social life into the academic world and will yield students that are more interested and involved in their studies. Graff acknowledges that there are limits to this approach just like any other. A quote from his peer, Ned Laff, summarizes the challenge in education. â€Å"[The challenge] is not simply to exploit students’ nonacademic interests, but to get them to see those interests through academic eyes† (Graff, 385). Being a cognoscenti on the top songs of the decade is not enough, one should be able draw relationships between the trends of the songs in order to come to a general conclusion or theory that is applicable to other areas of life. Graff is careful to not belittle the classic academic texts and subjects. He claims that educators should use the topics their students are interested in as a gateway to more challenging topics. If one can teach a student to think critically about the implications of performance enhancing drugs in today’s athletes, then the hard part in education is complete. Once a student knows how to think critically, they can apply that technique to whatever subject they want to study for the rest of their life. It gives the power and responsibility of teaching to the student themselves. Graff ends his argument by appealing to the readers logic. Suppose he is wrong, and allowing trending topics in the classroom does not in fact motivate the student to become a student of the world. What does the educator stand to lose in trying? Even if all they ever are interested in studying are the current stats of the Baltimore Ravens, â€Å"they are more literate and reflective than they would be otherwise† (Graff, 386). I believe Graff presented a strong and valid argument worthy of discussion. I am able to relate on many levels with Graff. Often I find myself more interested in my fantasy football team than my college courses. Previously, I had assumed time spent on fantasy football was time wasted, but this essay has allowed me to develop a different perspective. In order to have a successful team, I had to decide what players would complement each other; this required close analysis of their strengths and weaknesses. Memorizing and studying stats has allowed me to develop my statistical analysis skills, a skill that otherwise would be left unrefined. After each season, game, play, I have to reevaluate my thoughts on a player in order to take into account the new information. It has taught me that the greatest players make the same amount of novel mistakes as the worst players but repeat their errors only a fraction as often. If more of my teachers had exploited my love of sports earlier in my education, I might have a more accurate idea of my academic capabilities. Although I agree with the central tenets of Graff’s argument, I think he is overly critical and general of today’s educators. There are teachers who exploit every opportunity to intrigue their students, and they deserve to be acknowledged in any argument critiquing the current education system. These are teachers who go beyond the status quo of â€Å"teaching to the exam.† They are the educators who attempt to instill in their students a passion to learn, explore, and test the boundaries of their mind. It is also important to elaborate on my use of the word â€Å"teacher.† A teacher is not restricted to the classroom, but I have extended the term to encompass all one’s advisors, especially a child’s parents. I believe Graff would agree with my statement that a child’s parent plays a vital and irreplaceable role in the education of a child. Rather than discourage discussion of the popular topics, parents can facilitate classroom learning through critical discussion of subjects directly related to the youth’s life. â€Å"Hidden Intellectualism† details a not so unique argument in a way that is easy for the general public to sympathize with. It provides people with a justification for their â€Å"guilty pleasures† and encourages a more critical mindset no matter what you are doing. A Case Study relating to the Principles of Anaesthetic Practice In this assignment the key features of the pre-operative assessment, anaesthesia and airway management will be explored. I will be identifying a range of drugs that are used and will explain their behaviour and indicate the dosage. I will be reflecting on my own experience of checking an anaesthetic machine following the AAGBI guidelines, and explaining how to check anaesthetic equipment with regards to the AAGBI guidelines. I will also be reflecting on a patient undergoing anaesthetic on my placement. I have changed the name of my patient who I will be following through the anaesthesia for confidentiality reasons. Therefore I will be calling my patient Helen. The HPC code of conduct states that you must respect peoples right to confidentiality (Urwin 2007). Helen was referred to the hospital by her GP after suffering bad pains and tightening around her back. After a series of investigations which included an ultrasound scan, gastroscopy, MRI, and blood tests, it was found that Helen had stones in the gallbladder with increased wall thickness. This was detected through the ultrasound scan. She was then diagnosed with symptomatic gallstone disease and given a date to have Laparoscopic Cholecystectomy. For this procedure a general anaesthetic will be given and she will need to been intubated. Laparoscopic Cholecystectomy is the surgical removal of the gallbladder with a scope. These examinations where carried out in the out-patients consultation. Here the surgeon decided the diagnosis from the patients history, full physical examination and results from the investigations (Simpson, Peter 2002). A few days before the patient is due to come in for surgery they are asked to attend a pre operative assessment clinic. Here the patients general heath is checked, current and past medical history is asked and also any family history such as, hereditary conditions like malignant hyperthermia, cholinesterase abnormalities, and porphyries. These can all affect the anaesthetic process in some way (Aikenhead et al 2007). Bloods are then taken for cross matching and the anaesthetist requested an electro cardio gram to be taken due to Helens age. The patient is also asked about any medications she may be taking as these too can interact with some of the agents that may be used during anaesthesia. Alcohol, smoking and the use of recreational drugs can all affect the anaesthesia at some point. For example someone who drinks alcohol regularly may have liver dysfunction which may lead to the patient to be resistant to a range of drugs including induction drugs and sedative drugs (Davies and Ca shman 2004). The anaesthetic history of the patient will also be looked at, questions will be asked to the patient and anaesthetic notes will be read by the anaesthetist. This is carried out as the patient may have had problems during anaesthetic in the past and this could then be avoided (Aitkenhead ET all 2007). At the end of the assessment the patient has the chance to ask the anaesthetist any questions they may want to ask. This is great for the patient as it can limit any stress they may have and this in turn reduces blood pressure and relaxes the heart (Davey and Ince 2005). The pre operative assessment is also there to ensure that the patient knows exactly what procedure they are having (Wicker and ONeil 2006). The assessment identifies any potential problems which can save the operation from been cancelled (Simpson Peter 2002). Helen had no history of any conditions that ran in the family; she is currently taking no medications and has never had an anaesthetic before. The day before the operation the anaesthetist visits the patient on the ward. Here some final checks are done to see whether the patient is fit for surgery (Davey and Ince 2005). The patients physical status is assessed. This is done using the ASA classification. Class 1 being a healthy person and class 5 being a patient who is not expected to survive without an operation. Helen was class 2 which meant mild to moderate systemic disease not necessarily related to the condition requiring surgery, (e.g. hypertension). Helens physiological measurements were taken. Her Blood Pressure was 120/90, Respiratory Rate 18, Temperature 36.5, Body mass index 26, Oxygen saturation 96%, Pulse rate 70, Blood Glucose level 8.2, and weight 80.3kg. The patients airway is then assessed using the mallampati scoring system. This is done by the patient opening their mouth and sticking out their tongue. It is used to try and forecast how easy or difficult it will be to intubate the patient. The lower the sco re the easier to intubate (Steven et al 2003). See appendix 1. Helen has a mallampati score of 2 which meant that intubation shouldnt be too hard. However the anaesthetist also looks for other physical problems that may cause intubation to be difficult, for example, sunken cheeks, small mouth, prominent jaw, a short neck, and loose teeth (Woodhead and Wicker 2006). Helen had none of these problems however a difficult intubation trolley is always kept in the anaesthetic room incase intubation become difficult. If the patient is obese then they are usually advised to lose some weight before the operation as obesity increases the risk of wound infections, chest infections, and deep vein thrombosis (Davies and Cashman 2006). The patient should then be prepared for theatre by signing a consent form, fasting and pre medication. Fasting is where the patient cannot eat for 6 six hours, and not to drink any fluids after 2 hours before surgery (Woodhead and Wicker 2006). This is to reduce the risk of aspiration of stomach contents during induction or recovery of anaesthesia (Wicker and ONeil 2006). According to Davies and Cashman (2006), pre-medication is in place to relax a patient before they go down to theatre, and also to reduce any pain that the patient may be in. During the pre-operative assessment Helen was given 200mg of ibuprofen to be taken up to the surgery and on the morning of the operation. This was to reduce and pain that Helen may have been in. It is a mandatory requirement to check all anaesthetic machines and equipment before use each day. In 2004 the association of anaesthetists of Great Britain and Ireland published their third edition checklist of how to check anaesthetic equipment. The check list was approved by professionals and covers various aspects of checking the anaesthetic machine including pipelines, breathing system, ventilation, and monitoring equipment. Staffs have to be trained to check the equipment and a book must be signed by each person who checks the machine and equipment. (The association of anaesthetists of Great Britain and Ireland 2004). With the supervision of my mentor I started to check the anaesthetic machine ready for the day. I firstly made sure that the anaesthetic machine was connected to the mains electricity supply, and switched on. I then began to test all monitoring devices such as the pulse oximeter, capnograph, and oxygen analyser. I then checked that all pipelines were connected and connected to their correct terminal. This is to prevent the wrong gas been given to the patient. The pipe probes have a collar around them and each is different diameters to the other which prevents the probe being inserted into the wrong exit so the wrong gas in theory would never be given to a patient however it is still very important to check them. The check for this is known as the tug test. To do this I pushed the pipes is into the correct point and then i tried to tug them back out. This will tell you whether it is connected properly. The next piece of equipment on the anaesthetic machine that needs to be checked is the flow meter; i did this by looking to see if the bobbins were spinning and moving freely. I checked the anti-hypoxia device by turning the oxygen and nitrogen oxide on and disconnecting the oxygen pipeline. The nitrogen oxide should drop first and an alarm sounded, this is called the Bowson alarm. This told me that everyone was working how it should be. I then checked to make sure that the emergency bypass was working. I checked to see if the vaporises were securely fastened onto the machine, and did not leak. A then carried out a leak test on the breathing circuit and facemasks, and I checked that airways all the appropriate sizes. Then lastly I checked the ventilator for leaks and made sure that tubing was securely attached and the scavenging tubing was attached to the correct exhaust part of the breathing system, i then switched the gas on. Whist I was checking the anaesthetic machine I felt really nervous, I knew how important it was to check everything and everything correctly and this along with my mentor watching just added to the pressure. With the supervision of my mentor again I started to check the anaesthetic equipment before Helen was due for her operation. I made sure that I had out on the table 2 Macintosh laryngoscopes, one with a size 3 blades and the other with a size 4 blade. I then checked that the blades were securely fixed to the laryngoscope and also made sure that the light was working and bright. I then made sure that I had each size endotracheal tube out, a size 7, 8 and 9 and checked them by deflating the cuff and re-inflating it listening out for any leaks. I then got a size 3 and 4 LMA out and did the same test with these deflating and re-inflated but also looking for any holes or rips. I then checked that I had out all the equipment needed in case of difficult intubate, for example a gum elastic bougie, and Magill introducing forceps. The forceps are used to feed the endotracheal tube down into the trachea. I then made sure that I had a 50ml and a 20ml syringe, and also something to tie the endotr acheal tube in place and some swabs. We were then ready for Helen. On the day of the operation Helen was brought down to the theatre reception where the patient liaison asked Helen a few questions, got her to check that she had signed the consent form, and made sure that the correct site had been marked. The patient liaison then passed this information to me and my mentor in the anaesthetic room. I introduced myself to Helen and explained what i was going to do next. Whilst the anaesthetist drew up the drugs I put ECG pads onto Helen, a pulse oximeter onto her finger, and a blood pressure cuff onto her arm. The anaesthetist then inserted 0.5ml of 1% Lidocain onto the back of Helens hand where he wanted to insert a cannula. Lidocain is a local anaesthetic and was inserted to numb the area he intened to insert the cannula. It is stored in a lockable cupboard in the anaesthetic room (BNF 2009). The anaesthetist then inserted a cannula into a vein on the back of Helens hand. Helen was then pre oxygenated via a facemask which I held over her during induc tion. 10mg of morphine was firstly given via the cannula. 10mg of morphine is given as it produces better sedation and reduces the incidence of nausea and vomiting (Clarke et al 2005). 2mg/kg of Propofol (anaesthetic agent) and 100 ug/kg vecurionium bromide (muscle relaxant) was then inserted into the cannula. Propofol is a general anaesthetic and it works by slowing down the brain and nervous system. There are some side effects to Propofol, these are: pain/swelling at the site of insertion, weak shallow breathing, fast or slow heart rate, and some people can have an allergic reaction to the agent. Propofol is stored in a lockable cupboard which can only be accessed by the ODP in charge. Vecuronium Bromide is stored in a locked fridge. It is a muscle relaxant and works by blocking signals between your nerves and your muscles. This agent also has some side effects, these are: an allergic reaction to the drug. Also things such as weak/aching muscles, trouble breathing, and feeling lig ht headed. (BNF 2009). After the anaesthetic agents had been inserted it was time to intubate Helen. I assisted the anaesthetist along with my mentor through this process, passing the anaesthetist any intubating equipment he needed. He gently lifted her head and inserted a Macintosh laryngoscope with a size 3 blade into Helens mouth so that he could get a view of the trachea. He then placed a size 8 tracheal tube into the trachea. I inflated the tube listening for escaping air. Helen was now successfully intubated. To secure the endotracheal tube in place I tied a bandage around the tube. I then placed an upper body warmer onto Helen to maintain her temperature throughout the procedure and also some flowtron boots to prevent pressure sores. Intermitted positive pressure ventilation (IPPV) was used throughout Helens operation and because of this an airway pressure monitor was used. Helen was then maintained on 1litre oxygen with 1litre of nitrous oxide through a closed circuit with a soda lime canister to remove the carbon dioxide. Throughout the surgery the muscle relaxant was maintained by vecronium bromide, and carbon dioxide levels were kept at 35-45 mmHG. Due to Helen having a general anaesthetic she needed to be reversed with 50ug/kg neostigmine with 10ug/kg glycopyrrolate. Once Helen had been reversed from the vecromium and seen to be breathing on her own she was extubated. Firstly her mouth and the back of her throat were cleared of any secretions using suction. This is to prevent any secretions going down the trachea compromising the lungs which could lead to difficulty with breathing and infection (Gardiner and Grindrod 2005) The endotracheal tube cuff was then deflated and with the aid of a laryngoscope the tube was removed and a gadel airway was inserted to aid breathing and to prevent the tongue from falling back and causing choking. The monitoring was then removed and a mask was applied with 5litre oxygen to keep the patient oxygenised whilst bein g transferred to the recovery ward. Monitoring that was used throughout the anaesthesia and the surgery itself was the electrocardiogram, non-invasive arterial pressure monitor airway pressure monitor, pulse oximeter, end-tidal carbon dioxide concentration monitor, peripheral nerve stimulation and body temperature probe. Helen was very nervous when she got into the anaesthetic room. To try and reduce this I communicated with Helen and held her hand through the insertion of the cannula.
